Salford's Intermediate care service sits under the newly formed Integrated Care Organisation.
Integrated care is all about providing better care and support to our patients, users and the people of Salford by co-ordinating our services more effectively, with the focus on improving the health and wellbeing of people across the city.
Salford Together is a partnership between NHS Salford Clinical Commissioning Group,Salford City Council, Salford Royal and Greater Manchester West Mental Health NHS Foundation Trust.
Together, this partnership is working to integrate health and social care for older people in Salford, bringing the contributions of GPs, District Nurses, social workers, mental health professionals, care homes, voluntary organisations and local hospitals into a more aligned system and provide older people with the support they need to manage their own care.
As the team lead of one of 3 intermediate care rehab units you will be expected to lead the provision of a clinically effective and cost efficient service and clinically lead a dedicated team of therapists and support staff.
You will also provide clinical and developmental support to junior physio staff within the other units.
You will be responsible for the development and delivery of a consistently high quality service and taking and active part in providing direct patient care and acting as an expert practitioner and a role model to the whole team.
It is essential that you can use your intiative and work without direct supervision.
You will have a minimum of 4 years post registration experience including 2 years working in rehabilitation with adults and older people.
Due to the nature of community working you will be required to be a driver and have access to a vehicle for work purposes.
