An exciting opportunity has arisen for a Band 6 Registered Nurse to join the Community Discharge Co-ordination Team (CDCC) as a Case Manager, 30hrs per week.
The post offers varied and interesting work with people who require inpatient or community rehabilitation. You will work across both the acute hospitals in Bristol and the community. In the acute hospitals you will be involved in assessing patients requiring further rehabilitation and ensuring that they are placed in a suitable rehab setting and working alongside the Integrated Discharge Teams .
In the community you will be joining the team at an exciting stage as CDCC is involved in rolling out the new Discharge to Assess (D2A) programme where you will be promoting independence of older persons in their own homes and liaising with the community therapy teams. You will be working in a dynamic team of Case Managers, Social Care Practitioners and Assistants providing a vital service to the diverse population of Bristol.

You will need to have experience of working in acute and community settings; a knowledge of rehabilitation and services; good IT skills and flexible to work in a variety of locations across the city.

Bristol Community Health (BCH) provides a range of community services 365 days a year, over a 24 hour period. This may result in staff being required to work a variety of shift patterns.
