Coventry and Warwickshire Partnership Trust was formed in 2006 and became integrated with NHS Coventry in April 2011.
The Trust provides a full range of expanding physical, mental health and learning disability services to children, young adults, adults and older adults over many sites across Coventry, Warwickshire and Solihull.
Services are provided to a population of over one million people living in Coventry and Warwickshire and also a wider geographical area in some of our specialist services, we see on average around 5,000 patients every day.

We have an opportunity for a Band 5 Nurse to join the South Warwickshire Memory Assessment and Community Dementia Service IPU 18-21 (Integrated Practice Unit). This IPU offers assessment and evidence based interventions for people of who are undergoing memory assessment or who have an existing diagnosis of dementia. The team is proactive in embedding and using multidisciplinary roles to provide support and outcome focused interventions.
We need someone who is motivated and passionate about working with this client group and you will be expected to care coordinate a caseload as well as contributing to the development of the IPU. In addition to this you should also be able to demonstrate that you can work effectively as part of a multi-disciplinary team along with other Health, Social Care and Third Sector organisations.
You should be able to demonstrate good verbal and written communication skills and exhibit behaviours that reflect Coventry and Warwickshire Partnership Trust's vision and values.
The successful candidate will receive managerial and professional supervision from their line manager and peers within the IPU, as well as CPD opportunities within a culture that encourages the development of its staff.
